Hit the "Login" button towards the bottom-left of the phone screen. See red arrow below.
Key in your extension then press the down arrow as indicated by the left picture. Then the enter the password 5555 and press the down arrow. Enter the desk type as 1 then press the Done button.
Your screen should look similar to the right picture, except with your extension typed in. Once you've hit the Done button your phone will reboot, and you will be logged in.
